Abstract

This study investigates the diagnostic boundaries between autism spectrum disorder (ASD), bipolar affective disorder, schizophrenia, and giftedness/high abilities, focusing on symptom-by-symptom analysis, clinical and organizational psychopathology, and identifying more assertive interpretations of speech and behaviors frequently confused with psychopathological conditions. The central problem lies in the growing incidence of imprecise psychiatric diagnoses, often based on fragmented assessments, without considering traits of giftedness or social and communicative contexts. The main objective is to propose more accurate and interdisciplinary diagnostic and prognostic guidelines, contributing to the refutation of inadequate medical reports and the strengthening of the legal guarantees of the subjects. The research adopts the gifted neoperspectivist paradigm, based on premises such as the coexistence of absolute and relative truth, self-management, epistemological humility, and the appreciation of diversity and justice. Theories such as the Theory of Mental Codes, the Theory of Historical-Cultural Subjectivity and the Purpose-Oriented Systemic-Dialectic Theory supported the analysis. The hypothetical-deductive method was used in all stages, combined with a scientific bibliographic and documentary narrative review, with rigorous selection criteria. The Medical Hypotheses and Revista da Associação Médica Brasileira databases were consulted, using descriptors such as giftedness, misdiagnosis, bipolar disorder, autism spectrum, overdiagnosis, psychiatric false positives and differential diagnosis, totaling 104 articles analyzed. The main findings indicate a high incidence of mistaken diagnoses and the need for more integrated protocols. The main conclusions point to relevant theoretical, methodological and empirical contributions, although with operational limitations and the need for culturally sensitive instruments. The research adds value to science, postgraduate education and social justice.
