Abstract

Cardiovascular disease (CVD) remains the leading global cause of mortality and disability, with more than 18 million deaths annually and persistent disparities affecting low‑ and middle‑income countries (LMICs). 1,2 Recent advances in vascular aging assessment, artificial intelligence (AI), multi-omics biomarkers, and digital health technologies have expanded opportunities for early detection and precision prevention. 3 This integrative review synthesizes updated evidence (2020–2026) on epidemiological trends, emerging diagnostic tools—including AI‑enhanced ECG and photoplethysmography (PPG), arterial stiffness indices, genomics, and self‑sampling technologies—and implementation strategies for scalable prevention. While technological innovation accelerates rapidly, equitable deployment remains a major challenge. 4 Strengthening primary care, improving data governance, and fostering global partnerships are essential to achieving meaningful reductions in premature CVD mortality.

Keywords: cardiovascular disease, global burden of disease, early detection technologies, artificial intelligence, precision prevention, low- and middle-income countries
